All forms and fees are due on or before July 21, 2008 (with the exception of the Deposit Slip and $150 deposit, as indicated below, due May 16). 

You may indicate your desire to set up a payment plan on your deposit slip. See below for payment instructions.

Ladies Concert Attire Order Form (for those ladies who need to order a black dress - indicate the need and pay on your Band Class Fee Sheet and turn in this form for sizing)
